The 2010-2011 membership forms are now posted.
Please Mail original documents to SHSRA -c/o Maxine
Lehmann -secretary, Box 56, Mayfair, SK. S0M1S0.
Forms that need to come to the office are:
NHSRA membership form/minors release
SHSRA membership form/principals signature page
Members Profile and Health form. 1 cheque for $125
for membership fee, 1 postdated(April 1st, 2011) cheque for $100 for
sponsorship fee, 1 volunteer cheque for $50. A copy
of your June, 2010 report card or transcript.
If all the above are not sent to the office, your membership
will not be processed. Please READ carefully and fill
in all the blanks. Back numbers and any papers that you
will need will be handed out at our 1st fall rodeo(for all members)
Renewing members you will keep your numbers, New members
and Junior members moving up to High School will receive a
new number. Any questions about how to fill out your
forms or other questions, please don't hesitate to call
